Black Friday Promotional Terms
Eligibility
- Offer valid for new and existing patients completing a Curbside telehealth evaluation.
- Patients must be physically located in New Jersey at the time of the telehealth visit.
- Patients must register online via www.curbside-ortho.com and consent to Curbside telehealth services.
Promotion Details & Insurance Verification
- Patients will pay no more than $50 for the promotional telehealth visit.
- For patients to qualify for the specialist co-pay equivalent price, they must submit verified documentation confirming their current specialist co-pay amount.
- Patients may upload documentation through the patient portal or present it to the Curbside provider during the virtual appointment. Curbside providers will review the documentation and confirm the final promotional pricing during the telehealth visit.
- If valid documentation is not provided, the visit will be charged at the promotional rate of $50.
Scope of Promotion
- The promotional price applies only to the telehealth evaluation.
- Additional services such as imaging, procedures, medications, or outside referrals are billed separately.
Scheduling & Availability
- Appointments must be scheduled and completed between November 28th , 2025 and December 5th 2025.
- Appointment availability may be limited.
No Cash Value & Non-Transferability
- Offer has no cash value and cannot be transferred, refunded, or combined with other promotions.

- By participating in this promotional visit, the patient acknowledges that they have read and agree to Curbside’s Terms of Use and to Patient Financial Policy.
